We’d love to hear how our community spent the grand final long weekend!

Were you watching the footy, cheering from home, enjoying a BBQ or proudly wearing your team’s colours? 🏈

And most importantly… who were you barracking for?

Our participants had a fantastic day out in Macclesfield on Saturday at our group activity, enjoying some fresh air, beautiful surroundings and a little fishing fun! From casting a line to enjoying lunch together and meeting the friendly farm animals, it was a wonderful day filled with connection and laughter. 💚

We love creating opportunities for our participants to get out, try something new and enjoy the community around them! 🌿✨

There’s something pretty special about the quiet moments.

Two people, a park bench, a little bit of nature and time to simply sit, connect and enjoy each other’s company.
Slowing down, being present, listening and creating genuine connection.

✨ Meet Alanah! ✨

It’s been a while since we’ve properly introduced this wonderful human!

Alanah is one of our superstar support workers and brings so much energy, creativity and heart to everything she does. She absolutely loves organising events, adding all those thoughtful finishing touches that make people feel special, and most of all... creating opportunities for fun!

Whether she’s planning an activity, decorating a space or making sure everyone feels included, Alanah has a knack for turning ordinary moments into memorable ones.

We’re so lucky to have you as part of the Lyft Community family, Alanah.

What does good care actually look like?

We think it looks like this. 👇

Someone who remembers your mum's favourite biscuits.
A worker who doesn't need to be told twice how you like your morning done.
A team that calls when something changes — not after.

Care isn't a service. It's a relationship.

At Lyft Community, we hire people who believe that. And we build rosters around it — so you see the same face, week after week.

Because trust takes time. And time matters.

Switching NDIS providers shouldn't feel like a battle. 🌿

But for a lot of participants, it does.

You finally find a provider you like, and then the worker changes. Or they can't do your hours. Or the communication just... stops.

At Lyft Community, we do things differently.

→ We match workers based on personality, language, and interests — not just availability
→ We're locally based on the Mornington Peninsula and cover South-East Melbourne

NDIS funded. Plan managed and self managed welcome.

If you're thinking about switching — or looking for your first provider — let's have a chat.

If your parent is on a Home Care Package — things have changed.

From November 2025, the Home Care Packages program was replaced by Support at Home. Same goal, new rules.

Here's what that means for families:

- More flexibility in how funding is used
- Clearer categories — Clinical Care, Independence Support, Everyday Living
- Quarterly budgets instead of annual packages
- You may need to review your current provider and service agreement

Not sure if your parent's care is set up correctly under the new system?

We offer a free 15-minute chat to help families understand what they're entitled to and whether their current support is still working for them.

No pressure. Just answers.

I started Lyft Community because I kept hearing the same thing from families.

"Mum had a different person every week."
"Dad didn't know who was coming."
"We just wanted someone who actually knew him."

Care should feel like care — not a roster of strangers working through a checklist.

So we built something different. Consistent support workers. Real relationships. A team that actually knows the people they support.

We're based on the Mornington Peninsula and cover South-East Melbourne. NDIS and aged care.

If this sounds like what you've been looking for, reach out. I'd love to talk.

Welcome to Lyft Community. 🌿

We're a home care provider based on the Mornington Peninsula, delivering NDIS support and aged care services across South-East Melbourne and the Peninsula.

What makes us different?

You'll have the same support worker. Not a roster of strangers, one person who learns your routine, remembers how you take your tea, and actually shows up.

We support older Australians through the new Support at Home program, and people living with disability through the NDIS.

If you're looking for support — for yourself, a parent, or someone you care for — we'd love to talk.
