ESR and CRP Overview

ESR (Erythrocyte Sedimentation Rate) and CRP (C-Reactive Protein) are common laboratory tests used to detect and monitor inflammation in the body. While both indicate inflammation, they differ in how quickly they respond and what they measure.

ESR (Erythrocyte Sedimentation Rate)

ESR measures the rate at which red blood cells settle to the bottom of a test tube in one hour. A faster sedimentation rate suggests the presence of inflammation.

Common Causes of Elevated ESR

Bacterial and viral infections

Autoimmune diseases (e.g., rheumatoid arthritis, lupus)

Chronic inflammatory conditions

Certain cancers

Anemia

Pregnancy

CRP (C-Reactive Protein)

CRP is a protein produced by the liver in response to inflammation. It rises rapidly during acute inflammation and decreases quickly as the inflammation resolves.

Common Causes of Elevated CRP

Acute bacterial infections

Autoimmune diseases

Tissue injury or trauma

Recent surgery

Cardiovascular inflammation

Important Note

ESR and CRP are non-specific tests. An elevated result indicates inflammation but does not identify the exact cause. They should always be interpreted together with the patient's clinical findings and other laboratory investigations.

High Density Lipoprotein (HDL) comprises a complex family of particles that vary in their lipid and protein composition. Based on density gradient ultracentrifugation, HDL can be of two distinct subtypes; HDL2 (larger and buoyant) and HDL3 (smaller and dense). HDL2 is formed in circulation from HDL3 through the action of lecithin-cholesterol acyl transferase (LCAT) enzyme and HDL2 contains twice as many cholesterol molecules per unit of apolipoprotein compared to HDL3. These HDL subclasses are believed to play different roles in arteriosclerosis progression and regression. HDL2 has been suggested to be the more variable component of total HDL and several lines of evidence showed that it is the more anti-atherogenic form of HDL. Therefore, decreased levels of HDL2 cholesterol is associated with an increase in the risk of atherosclerotic cardiovascular disease.
