High Density Lipoprotein (HDL) comprises a complex family of particles that vary in their lipid and protein composition. Based on density gradient ultracentrifugation, HDL can be of two distinct subtypes; HDL2 (larger and buoyant) and HDL3 (smaller and dense). HDL2 is formed in circulation from HDL3 through the action of lecithin-cholesterol acyl transferase (LCAT) enzyme and HDL2 contains twice as many cholesterol molecules per unit of apolipoprotein compared to HDL3. These HDL subclasses are believed to play different roles in arteriosclerosis progression and regression. HDL2 has been suggested to be the more variable component of total HDL and several lines of evidence showed that it is the more anti-atherogenic form of HDL. Therefore, decreased levels of HDL2 cholesterol is associated with an increase in the risk of atherosclerotic cardiovascular disease.
