• TIN ID o Tax Identification Number
• Milling Statement (mula sa URC / ProGreen)
• RSBSA Registration Number
• Affidavit of Undertaking
• 2 Valid ID at Planter ID (photocopy na may 3 specimen signatures)
• 2 pirasong 1x1 ID picture
• 1 Co-borrower o 2 Co-borrower (kung ang borrower ay 60 taong gulang pataas)

* 2 Valid ID (photocopy na may 3 specimen signatures)
* Marriage Contract (kung may asawa)

Awarding of OPSI and FLT Certificates to participants.

IN PHOTOS: In August 2-4, 2023. The SRA Balayan Mill District conducted an Outreach Program for the Sugar Industry (OPSI) and Financial Literacy Training for 30 borrowers of the SIDA-Socialized Credit Program, which was held at the Barangay Hall of Brgy. Sucol, Balayan, Batangas.

The OPSI was conducted on the first and second days of training. Mr. John Michael Basabe, Project Evaluation Officer I, started the program with a warm welcome on the participants. The first two topics are Sugarcane Plant and Soil Sampling, were discussed by Ms. Jessa Dorado. It was then followed by a lecture on land preparation and planting by Ms. Joan Caviteño. Mr. Michael Kyle Romulo talked about Pest and Diseases Management and Crop Maintenance Management. The last topic, Plant Nutrition and Harvesting, was discussed by Mr. John Michael Basabe. Mr. Joel Ronario, SRA Balayan MDO, gave some additional information regarding the proper application of herbicide and calibration using Knapsack Sprayer. He also gave his inspirational message to the borrowers and explained the importance of the training.

For Day 2 and 3, the modules in Financial Literacy Training (FLT) was discussed by Mr. Michael Kyle Romulo. The FLT program contains four modules entitled: Personal Financial Management, Savings, Loan and Credit Management, and Investment.

The OPSI and FLT were effectively administered as the borrowers showed their appreciation for the new sugarcane technologies and financial literacy that were introduced to them.
