Original OSCA ID/NATIONAL ID
Para sa mga Deceased Representatives
✅2 Copies of Death Certificate from C/LCR (1 Original Certified True Copy from C/LCR and 1 photo copy
✅2 photocopies of valid of back- to- back of the Representative
✅ 2 photocopies of valid ID(OSCA ID) back-to-back of the decease beneficiary
✅2 copies of warranty and release from.liability (1 original and 1 photo copy)
