From the Fun Run and Zumba Kickโ€‘Off, to the Cleanโ€‘Up Drive, and culminating in the grand events of Silnag ken Baggak ti ISMC 2026 and Himig ng Lingkod Bayani, this journey was marked by camaraderie and excellence. When the ISMC family unites, we achieve remarkable things, showcasing talent, beauty, and our passion for public service.

Our appreciation goes to the overall committee led by Dr. Sebastian Isagani Moll Jr., the Fun Run and Zumba sub-committee chaired by Ms. Myra Gazmen, the Cleanโ€‘Up Drive sub-committee chaired by Engr. Kevin Etrata, the PRAISE Committee chaired by Ms. Marites Guieb, the Pageant/Organizing sub-committee chaired by Ms. Xai Ducusin, and all committee members whose commitment made this celebration possible.

We also honor our candidates, participants, and awardees whose presence and contributions brought vibrance, inspiration, and meaning to this milestone.

To our Division Chiefs - Dr. Karla Mae Briones, Dr. Sebastian Isagani Moll Jr., Ms Myra Gazmen and Ms. Mary Joyce Haban, most especially to our Medical Center Chief, Dr. Angel Trinidad III, thank you for your unwavering support and vision.

This celebration reminds us that the spirit of camaraderie and service lives within us. May the energy and inspiration we found this month continue to guide us in serving with love, excellence, and dedication.

18 (EIGHTEEN) UTILITY WORKER

JOB APPLICATION REQUIREMENTS:

Interested and qualified shall submit the following to the Human Resource Management Section of Ilocos Sur Medical Center:

1. Application letter addressed to the Medical Center Chief II, ANGEL D. TRINIDAD III, MD, FPCP, MHA (please indicate the Position Title of the vacant position you wish to apply for)
2. Fully accomplished and Notarized Personal Data Sheet (PDS) with Work Experience Sheet and recent passport-sized or unfiltered digital picture (CS Form No. 212, Revised 2025); wet signature or digitally signed or electronically signed;
3. Hard copy or electronic copy of the Performance rating in the last rating period (if applicable);
4. Hard copy or electronic copy of proof of eligibility/rating/license (if applicable);
5. Hard copy or electronic copy of Authenticated Transcript of Records and Diploma (if applicable);
6. Certificate/s of Employment (if applicable) and
7. Photocopy of Certificates of Training
