To ensure a smooth and organized registration process, kindly prepare and submit the following requirements:

1. Voter’s Certificate

1.1 For participants aged 17 and below:
Attach the Birth Certificate and parent’s Voter’s Certificate

1.2 For participants aged 17 and below living with a guardian:
Attach a valid Affidavit of Guardianship.

2. Team Gallery / Player Gallery

Submit the official Team/Player Gallery complete with:

- Full names
- 1×1 ID photos
- Jersey numbers

3. Team Waiver / Player Waiver
(Accessible through the link provided below)
Accomplish and submit the required waiver form for all participating teams or individual players.

4. BOND FEE of One Hundred Pesos per head (PHP 100.00)
