Requirements Upon Claiming:
1. Document Owner - Valid ID
2. Representative - Authorization Letter or PSA ( specific as to purpose and documents / requested. Photocopy of Valid ID of the document owner and his Representative.
3. Guardian of Minor - Duly authorized Affidavit of Guardianship Barangay Certification confirming the guardianship of the minor children. Photocopy of Valid ID of the Guardian.
