The ideal candidate will serve as a support to multiple community-based organizations employing Community Health Workers contracted to provide care coordination through the HOPE Pathways Community HUB. The Program Administrator coordinates the administrative duties in building, implementing, and sustaining the HOPE Pathways Community HUB in Kings County.
