Previously, she held leadership roles with Kaiser Permanente and the Robert Wood Johnson Foundation, helping shape research, policy, and community health initiatives nationwide.

Come work with us! We are seeking a Program Administrator in Kings County with strong organizational skills and attention to detail. The ideal candidate will serve as a support to multiple community-based organizations employing Community Health Workers contracted to provide care coordination through the HOPE Pathways Community HUB. The Program Administrator coordinates the administrative duties in building, implementing, and sustaining the HOPE Pathways Community HUB in Kings County.

The Care Coordination Manager is a seasoned, frontline public health worker who is a trusted member and has a remarkable understanding of the community within Kings County. They will serve as a direct supervisor to a project administrator and a support to multiple community-based organizations employing Community Health Workers contracted to provide care coordination through the HOPE Pathways Community HUB. The Care Coordination Manager coordinates, providing outreach, education, and access to resources.

The perfect candidate will have a proven track record with coordinating, managing, and monitoring quality assurance projects, including but not limited to, building quality metrics, performing analysis, auditing, and providing reports related to the HOPE PCH. This entrepreneurial, self-starter, and collaborator will support the Care Coordination Manager and the team in building, implementing, certifying, and sustaining the HOPE Pathways Community HUB in Fresno and Kings County.
