For 1.8 billion people around the world, menstruation is a part of life, and yet it remains a hidden public health crisis. Stigma and inequities continue to prevent women and girls from fully participating in school, sports, work, and daily life.
And, too many people, especially young people, still lack access to the information, products, clean water, sanitation facilities, and health services they need to manage their health with dignity.

PAI joined dozens of organizations in urging Congress to include stronger fiscal guardrails in FY2027 appropriations bills to ensure the administration complies with the law and cannot unlawfully withhold, delay, or redirect funding passed by Congress.
The administration must not ignore congressional intent when it comes to appropriated funding. Given the recent attacks to critical global health and family planning programs, congressional oversight and accountability matter more than ever.

House Republicans are once again targeting international family planning and reproductive health programs in the FY2027 foreign assistance bill.
The proposal would:
🚫 Cut international FP/RH funding by 24%
🚫 Eliminate U.S. funding for UNFPA
🚫 Codify the dramatically expanded Global Gag Rule
🚫 Expand “culture war” restrictions through foreign assistance policy
✅ Require the Secretary of State to “promptly inform” lawmakers of the diversion or destruction of U.S. government assistance commodities

The Trump administration’s latest budget proposal would eliminate all U.S. funding for international family planning and reproductive health (FP/RH).
This part of a broader pattern:
🚫 Rescinding $500 million in FP/RH funding
🚫 Letting ~$10 million in contraceptives expire
🚫 Defunding United Nations Population Fund
🚫 Expanding the Global Gag Rule beyond abortion
These actions represent a systematic rollback of U.S. leadership in global reproductive health.
Access to contraception is one of the most effective ways to improve health, expand opportunity, and support women and families worldwide. Walking away from that investment will have harmful consequences.
Congress has rejected proposals like this before. The question now is whether it will do so again and uphold a six-decade commitment to women’s health and rights worldwide.

Today, President Trump released his budget request for fiscal year 2027, proposing dramatic cuts to global health, and made note that the administration does not believe U.S. funding should support “unfettered access to birth control.” This proposal is part of a continuing assault on contraceptive services and represents a threat to the health and lives of women and families worldwide.
“This budget proposal is both a betrayal of decades of American leadership and a direct threat to the women and families whose lives greatly benefit from U.S. support for family planning and maternal health,” said PAI’s Senior Fellow, U.S. Government Relations, Craig Lasher. “Gutting this funding does not reflect American values, nor does it serve American interests. Congress has a long history of rightly treating international family planning as a bipartisan priority, and we urge them to do so again.”
