Advance care planning is the process of detailing your future medical care preferences. It provides guidance to your family, close friends and healthcare providers if you can no longer speak for yourself or make your own decisions.
By adding an advance care planning document to My Health Record, your wishes can be safely and securely accessed if they were ever needed in a medical emergency.

The Agency’s Chief Clinical Adviser (Medicine), Dr Amandeep Hansra spoke to Guardian Australia recently about the role and other digital tools play in accelerating the diagnostic process and empowering her patients to manage their health more proactively.
“Knowledge is power,” Dr Hansra said. “Having access to information about your own healthcare helps you be more proactive about it, helps you be more involved and, ultimately, feel more in control.”
