Post-Discharge Homecare Workers
📍 North & East Belfast
💷 £13.80 per hour + mileage allowance
⏰ Full-time and Part-time Hours Available
Make a real difference in your local community by helping people recover safely and confidently at home following discharge from hospital.
As a Post-Discharge Homecare Worker, you will provide short-term support to individuals, promoting independence and helping to reduce the risk of hospital readmission.
Your role will include:
✅ Assisting with personal care, including washing, dressing and toileting
✅ Supporting mobility and safe movement as directed by healthcare professionals
✅ Helping with meal preparation and nutritional support
✅ Assisting with shopping and light household tasks
✅ Providing medication reminders in line with company policies and procedures
✅ Offering companionship, reassurance and emotional support during recovery

Caring, compassionate and reliable individuals
Excellent communication and interpersonal skills
A commitment to delivering high-quality person-centred care
Previous care experience is desirable but not essential, as full training will be provided
