60 seconds, three field observations on chemical safety:

Ask a worker what's in their container and what it reacts badly with. Their hesitation tells you more than any audit record.
Decanted containers are the blind spot. The original drum is labelled; the smaller container filled from it for daily use often isn't.
Storage by convenience, not compatibility, is the default unless someone organises it on purpose.

Nine questions to test your chemical storage and handling:

Can workers describe the main hazards without checking the SDS? Is storage arranged by compatibility, not convenience? Is every container labelled, including decanted ones? Is spill equipment specific to what's on the shelf? Has the inventory been reviewed in the last year?

Most sites pass on documentation and fail on the shelf.

An authorised safety-system override is not the same as a controlled risk.

In process industries, protective functions may occasionally need to be taken out of service for troubleshooting or maintenance. The danger lies in treating the authorisation as the end of the safety process.

A robust override-management process should establish:

Why the override is necessary.

What risk is introduced by losing the protective function.

Which compensating measures are required.

Who owns the override and how long it may remain active.

How restoration and functional testing will be verified.

Temporary conditions need active management. They should never become invisible parts of normal operations.

The objective is not merely to authorise the override. It is to control the risk until protection is fully restored.

Every drum on site has a safety data sheet filed somewhere. Very few of the people handling it have actually read theirs.

Chemical incidents rarely come from exotic substances. They come from ordinary ones stored badly: an oxidiser next to a flammable, incompatible wastes mixed together, a decanted container with no label at all.

Why it happens: SDS files are kept for audits, not read by the people doing the work. Storage follows available space, not compatibility. Secondary containers, the ones people actually use, often have no label.

A safer decision for one team can create a new risk for another.

In complex industrial operations, hazards do not always remain within the boundaries of a single task or department.

A maintenance change can affect operating conditions. A revised work sequence can alter another team's exposure. And two individually approved activities can still create an unsafe interaction.

The answer is not to blame individuals. It is to strengthen the system:

Assess risks across team and equipment interfaces.

Communicate changes to every affected group.

Revalidate controls when operating conditions change.

Confirm that the combined work remains safe.

Process safety is not just about controlling individual tasks. It is about understanding how those tasks interact.

60 seconds, three field observations on machine guarding:

Look for improvisation. Cable ties, tape and wedges around a guard mean it stood between a worker and finishing the job, and lost.
Ask operators which guards are "annoying". They already know exactly which ones slow them down.
Check what happens after a breakdown. Guards defeated during emergency repairs are the ones most often left that way.

Nine questions to test your machine guards. Walk your line and check:

Does every interlock actually stop the machine when tested? Any cable ties, tape or wedges on a guard? Can operators clear jams without defeating a guard? Does a bypass trigger an alarm? Are technicians required to restore guards before restart? Has anyone reviewed near misses for guarding failures?

Most lines pass a visual check and fail a function test.

An interlock that's been taped shut, wedged open or bypassed isn't a weaker guard. It's no guard at all.

Walk most machine shops and you'll find one: a cable-tied limit switch, a covered light curtain sensor, a gate interlock defeated during a breakdown and never restored. Each one was usually justified by a production target that couldn't wait.

Why it keeps happening: guards aren't designed for jam-clearing or changeovers, defeats are easy and silent, maintenance restores speed before restoring the guard, and audits check that guards exist, not that they still work.

Many people hurt in these incidents are rescuers who went in untrained, because waiting felt impossible. Plan rescue so nobody has to make that choice.
"Tested" and "tested where it matters" are different. A clean reading at the hatch can hide gas settled at the bottom of the vessel.
The attendant isn't a formality. Their job is full attention, for the full duration, every time.
