ANUNSYO

Makibasa po na maigi ang kailangan dalhin sa pag koleksyon ng pasaporte lalo na po sa pag koleksyon ng pasaporte ng baby. Hindi po namin maaari i-release ang pasaporte kung hindi dala ang kaukulang dokumento. Ito po ay base sa kautusan ng Department of Foreign Affairs sa pag koleksyon ng pasaporte.

Collection days: Monday to Thursday / 8:30 a.m.-12 p.m. except public holidays. No need to book an appointment.

Kasama po ang mga passports ng mga nag renew at newborn babies noong nakaraan na consular outreach mission ng Philippine Embassy in Rome dito sa Malta noong February 20 - 22.

Kailangan dalin sa pag claim ng passport (renewal)
- Original issued receipt
- Original valid ID
- Old passport

PARA SA MENOR DE EDAD NA APLIKANTE:

Kung INA ang kukuha ng pasaporte ng bata:
- Original issued receipt
- Valid ID ng INA na kukuha ng pasaporte ng bata

Kung AMA ang kukuha ng pasaporte ng bata:
- Original issued receipt
- Valid ID ng Ama na kukuha ng pasaporte ng bata
- Copy of Birth Certificate issued by the Philippine Statistics
Authority (PSA) or PSA document indicating proof of paternity over the minor child.
- Kung wala pang PSA Birth Certificate ang bata ay ang INA na lamang po ang mag-claim ng passport ng bata.

Kung applikate ay hindi makukuha ang kanyang pasaporte ng personal, ang kanyang immediate family members (asawa, anak, magulang o kapatid) ay maaaring kuhanin ang passport ng aplikante. Dalin lamang ang sumusunod na dokumento:

- Authorization letter (isulat ang relasyon/kaugnayan sa aplikante)
- Original passport ng aplikante
- Original issued receipt
- Valid ID (original and photocopy) ng aplikante at representative

Kung hindi naman po immediate family ng aplikante ang kukuha ng pasaporte ay kailangan magbigay ng sumusunod na mga dokumento:
- Original passport ng aplikante
- Original issued receipt
- Original notarized Special Power of Attorney (SPA) executed by the applicant, issued to the authorized representative
- Valid ID (original and photocopy) of applicant and representative indicated in the SPA.
