FOR BUILDING and OTHER STRUCTURES
- Sworn Statement of the market value of the property (i.e., SS) (3 copies)
- Copy of approved building permit, building plan, and / or Certificate of Completion or Certificate of Occupancy (2 copies)
- Bill of Materials (If necessary)
- Penalty of late filing of SS (If failed to file within the 60 days prescribed period)
- Current year Cedula (i.e., CTC)

FOR MACHINERIES
- Sworn Statement of the market value of the property (i.e., SS) (3 copies)
- Copy of Official Receipts / Deed of Sale to support declared costs and market values
- Penalty for late filing of SS (If failed to file within the 60 days prescribed period.)
- Current year Cedula (i.e., CTC)

FOR OTHER IMPROVEMENTS (Perennial Trees / Plants)
- Sworn Statement of the market value of the property (i.e., SS) (3 copies)
- Certification of the Barangay Captain as to introduced improvements
- Penalty for late filing of SS (If failed to file within the 60 days prescribed period.)
- Current year Cedula (i.e., CTC)

Immediate submission of the above documents will facilitate action of this office on the matter.

- Deed of Absolute Sale or any Deed of Conveyance (2 photo copies)
- Tax Clearance (Current year)
- Proof of payment of Transfer Tax ( Except DAR titled properties)
- Certificate Authorizing Registration (CAR) from BIR, if necessary
- Sworn Statement (i.e., SS) (3 copies)
- Proof of payment of penalty for late filing of SS (For failure to file within 60 days prescribed period)
- Current year Cedula (i.e., CTC)

FOR UNTITLED PROPERTY
- Deed of conveyance registered with office of the Register of Deeds
- Approved survey plan (2 copies if necessary)
- Tax Clearance
- Proof of payment of Transfer Tax
- Certificate Authorizing Registration (CAR)
- Sworn Statement (i.e., SS, 3 copies)
- Current year Cedula (i.e., CTC)
- Proof of payment of penalty for late filing of SS (For failure to file within 60 days prescribed period.)
