● Senior Citizen’s ID (from OSCA or LGU)
● Solo Parent’s ID
● PWD ID (issued by NCDA)

● Marriage Certificate (para sa mga kasal)

👶 Para sa mga MINOR DE-EDAD:
● Kailangang samahan ng magulang o guardian sa pagpaparehistro.
