1. 1 authenticated copy of Certificate of Registration (COR) with the signature of the authorized personnel from your school and a clear date of issuance. (Any incomplete document will not be accepted)

2. 1 copy of Brgy. Indigency Certificate with the ANNUAL INCOME details. (Combined household income of both parents should not exceed 400k but not less than 60k.)

3. 2 pieces of 2x2 ID pictures
