Establishments who engage in Food and Non-Food businesses is required to secure Sanitary Permit from the Municipal Health Office. Service available Monday to Friday 8am-5pm.
ISSUANCE OF SANITARY PERMIT
Requirements:
1. Duly accomplished Application Form - Sanitation Office
2. Barangay Business Permit - Barangay Hall
3. For renewal, previous Sanitary Permit - Sanitation Office
4. Water bacteriological analysis result - Sanitary Inspector's Office
5. Environmental Health and Sanitation Form 103-A with passing rate of at least satisfactory rating of sanitation standard. - Sanitary Inspector's Office.
STEPS
1. Proceed to Sanitation Office and present the requirements.
a. Check and review the requirements. (For positive result, refer to physician for proper management)
b. Release order of payment and instruct client to proceed to cashier.
2. Proceed to cashier and present the order of payment.
a. Receive payment and issue Official receipt.
3. Return to Sanitation Office and present Official receipt.
a. Check the Official receipt.
b. Release signed Sanitary Permit

ISSUANCE OF EXHUMATION PERMIT
Requirements:
1. Death Certificate;
For non-dangerous communicable disease, permit may be granted such bodies have been buried for period of 3 years. For dangerous communicable disease, 5 years. Permit may be granted for shorter time in special case such as requested by court, police, NBI subject to the approval for the regional director concerned.
2. For special cases letter request by the Court, Police/ NBI (Medico-Legal Cases).

Burial permit is a requirement to all who will apply for burial of dead person/s who will be buried in Alubijid both in public cemetery and private memorial parks.
ISSUANCE OF BURIAL PERMIT
Requirements:
1. Death Certificate
2. Certificate of Indigency (for indigents only)
3. From other places (permit to transfer Cadaver from his/her death place)

1. Present the requirements and pay the required fees to the revenue collection -
a. Check the documents as to authenticity and completeness.
b. Assess, receive fees, and issue official receipt
c. Prepare and release the burial permit.
