1. Photocopy of Enrollment Form or Certificate of Enrollment (COE) for 2nd Semester, SY 2025–2026
2. Original Barangay Indigency for “Educational Assistance” purposes
3. One (1) photocopy of any valid ID or School ID with three (3) original signatures

For scholars who are unable to personally claim their assistance, an authorized representative may claim on their behalf, provided they present an authorization letter together with all the required documents.
