Requirements:
-certified true copy of grade 6 form 138 (Photocopied, scanned or clear photo)
-complete 1st and 2nd grading grades
- grades in Science, Math, and English must be at least 85 and 83 in other subjects.
-NSO/PSA copy of birth certificate
