In accordance with the City HRMO's new memorandum, in hiring Job Order and Contract of Services, all applicants must submit the following documents effective September 15, 2022, to wit:

1. Request form for New JO and COS
2. Personal Data Sheet (notarized, with picture)
3. Personnel Inventory Form (with 2x2 picture)
4. Transcript of Records / Form 137
5. Birth Certificate
6. Medical Certificate
