Please be advised of the upcoming medical and registration activity in Kiangan:

What to Bring for PhilHealth Yakap Registration:

For NEW MEMBER:
A. 21-59 y/o:
Birth Certificate

B. SENIOR CITIZEN (60y/o and above):
National ID (philsys ID) or
Senior Citizen I.D

For DEPENDENTS:
*Ages 0–20;
- Birth Certificate
*Husband/ wife:
- Birth certificate
- Marriage Certificate

👂EVERYONE should be registered to the PHILHEALTH YaKAP Program inorder to avail of the comprehensive primary healthcare service:

​FREE PRIMARY CONSULTATION (KONSULTA) at the RHU: Free medical checkups, health risk assessments, and consultations with a primary care doctor at the Kiangan RHU.

​FREE PRESCRIPTION MEDICINE (GAMOT): 54 essential and maintenance medications in the YaKAP list of medicines that are prescribed at the RHU during KONSULTA will be availed for free at accredited pharmacies up to a limit of ₱20,000 per member/year.

​FREE OUTPATIENT LABORATORY TESTS at the Kiangan-RHU.

👀👉for PHILHEALTH members, you may register yourselves and your dependents to the YaKAP facility (KIANGAN RHU) thru the ff:
​Register: Sign up through the eGovPH App, the PhilHealth Member Portal, or directly at any PhilHealth-accredited YAKAP clinic.

​First Patient Encounter (FPE): Visit your chosen YAKAP clinic for an initial assessment, profiling, and ID verification.
​Avail Services: Use your YAKAP doctor's digital prescriptions/referrals to medications at partner facilities.
