This is a gentle reminder in relation to our upcoming 2nd Tranche of UPLIFT Assistance that will be held on September 28- October 1, 2026 at
Libmanan Cultural Center, Church Station Site, Bagumbayan, Libmanan
*Please be informed to our Beneficiaries who are included in the list regarding on their payout schedule ahead of time
*Kung sino po ang naka-claim nung 1st Tranche, sila lang din po ang mag-rereceive/claim for 2nd Tranche
*No representative and change claimant allowed unless if namatay po yung beneficiary na nag claim nung first tranche, in that case, make sure po na ang maga claim ay kasama parin sa CBMS 2024 Family Foster, kindly provide also Death Certificate as proof/reference
*Since ito po ay 2nd Tranche na, priority na po natin ay National ID since informed naman po sila nung First Tranche palang na kailangan na po nila mag-apply for National ID as one of the most priority reqs. needed for UPLIFT Assistance either, white paper or Digital National ID is allowed if wala pa pong PVC or Physical National ID
*If the beneficiaries did all their best na makapag-apply for National ID pero still di parin nila nakuha ito na po yung chance na mag-a-allow na po tayo ng Ibang Government Issued ID but make sure na may Transaction Slip/Proof of Registration po silang i-pipresent saamin kasabay ng 1 valid Id po nila
Ito lang po yung allowed:
1. Passport
2.Driver’s License (not expired)
3. Philippine Postal ID
4. PRC ID
5. UMID
6. SSS ID
7. Philhealth ID
8. Senior Citizens ID
9. Voter’s ID/Voter’s Cert (1 year validity only
10. PWD ID
Note:
National Police Clearance, and
Any Barangay ID’s are not Allowed.
*Kapag si Beneficiary po ay talagang walang National ID (any types of national ID), and walang Transaction Slip, wag narin muna po sila papapuntahin sa payout venue since hindi rin naman po sila mapa-process, subject for reschedule po sila but make sure po na sa reschedule date nila ay may National ID na po sila
•Regarding sa Reqs., Dala na po dapat nila ito prior pumasok sa venue para maiwasan po natin ang pabalik-balik since hassle pa po ito sa kanila lalo’t naka pila sila
-original and 3 copies of national id/govt issued id (yung isang photocopy ay wag na muna pong pipirmahan)
-original and 2 copies of proof of registration/transaction slip (need lang po nito kung hindi pa national id ang iprepresent)
*Claim stub
-Claim stub po must be provided before mag-start ang Payout, make sure na i-double check po ng beneficiary if tama po ang hawak nilang claim stub since doon po naka base ang Payroll No. nila
*If ang beneficiary ay aware na active na sila sa 4Ps and WGP mas okay po na wag na po silang papuntahin during the payout para hindi na po masayang ang oras nila.
During the actual payout ang dapat na dala lang po ni client ay Original ID, 3 photocopies of ID yung isa wala na muna pong pirma, Claim Stub and ballpen. Yan lang po Ma’am
