Ano-ano ang Libre?

1. Libre ang PSA Negative
2. Libre ang Notaryo sa Affidavit of Delayed Registration
3. Libre ang Notaryo sa Affidavit of Two Disinterested Person ( isasama lamang sa aming opisina ang mga pipirma upang doon pumirma)
4. Libre ang Affidavit to Use the Surname of the Father (kung kinakailangan)
5. Libre ang Affidavit of Acknowledgement/Admission of Paternity (kung kinakailangan)
6. Libre at kami na din po ang gagawa ng Certificate of Live Birth (Form 102)

Ano-ano ang kailangan Dalhin?

1. Dalawang (2) dokumento lamang na nagpapakita ng mga impormasyong ng iyong kapanganakan:

a) Basptismal Certificate
b) Form 137 or School Records showing facts of Birth
c) Medical Record (Immunization Card/Hospital Certificate) showing facts of Birth
d)Income Tax Return of Registrant or Registrant parents
e) Voter's Affidavit of ID(Certification)
f) Certificate Marriage
g) NBI Certification
h) Insurance Policy
i) SSS (Form E1)/GSIS of registrant or registrants; parent/s (if minor)
j) Any Public Document or ID of the registrant showing facts of birth

3. Certificate of Indigence (Brgy and MSWD)

4. Valid Id ng magpaparehistro.

5. Para sa mga ipinanganak sa bahay (2015 hanggang sa taong kasalukuyan) kinakailangan ng CLEARANCE from RHU (center).

Dear Honorable Mayor,

My name is Guillermo Mendoza, Professor Emeritus, University of Illinois. I am also a former professor at the UP College of Forestry and Natural Resources, UPLB.
I write just to let you know of my pleasant experience this morning with/at the Civil Registry Office. I needed/applied for a copy of our Marriage Certificate. I was met by a friendly receptionist at the door/entrance. Within minutes I was inside the office and was attended by an equally friendly ang courteous staff. After showing my ID and answering a few questions, he left /excused himself to get something. Within 2-3 minutes he was back and gave me a local/photocopy of my Marriage Certificate!
Wow! what a quick service! I was fully expecting to file my request and wait a few days or even weeks before I can get the copy! Regrettably, this is what I came to expect because of the well known 'bureaucracy'. Imagine my pleasant surprise when I get the copy...at NO CHARGE! Wow!
I write because I want to share with you the good things and service your staff at the Civil Registry Office is doing/has done for me this morning.
It is probably not often that you receive a commendation letter like this. But I thought the office deserves it! Of course, this also reflects the good service your office provides under your leadership.
Apologies for the informality of this letter. I would have like to write you formally and send it via e-mail, but the secretary said this informal letter is OK as well!
It's been a great Monday morning for me. I hope this will be a great week for you as well!
