1258H: Arrived on scene. Assessed a 92-year-old female with a chief complaint of dizziness secondary to a fall. The patient denied vomiting, was conscious and coherent, and was ambulatory.
1301H: Transported (10-76) to Quezon City General Hospital for further management, as requested by a relative.
