1. Letter of Intent specifying the position applied for and the place of assignment
2. Duly accomplished Personal Data Sheet (PDS) - two (2) original copies of CS Form No. 212 revised 2017 with attached Work Experience Sheet and recent passport size picture with complete
name tag and signature over printed name. Picture must be taken within the last six (6) months. Computer generated or photocopied picture is not accepted.
3. Authenticated copy of Certificate of Eligibility, Board/Bar Rating and License - two (2) authenticated copies each
4. Authenticated copy of Transcript of Records (TOR)
5. Certified True Copy of College Diploma
6. Original Certificate of Live Birth
7. Original copy of Certificate of Marriage (if married)
8. Original copy of NBI Clearance
9. Original copy of Court Clearances from RTC and MTC and Clearance from the Office of the Prosecutor where applicant resides
10. Performance Rating in the last rating period (if applicable)
11. Latest and duly filed Income Tax Return (ITR/VV2) (if employed)
12. Latest and duly filed Statement of Assets Liabilities and Networth (SALN) (if Government Employee)
13. Two (2) pieces 2x2 colored ID picture w/ white background (Note: Without make-up and filter, in business attire)
