2pcs. 2x2 picture (recent picture)
NSO birth certificate (photocopy)
Certificate of Unemployment/Employment of parents (photocopy)
Income Tax Return (photocopy)
Grade 10 card (photocopy)

Kindly put all of your requirements in a long brown envelope and submit it at the Guidance Office or at the Registrar's Office look for Ms. Myra

Schedule for the entrance exam (Grade 11) will be posted soon.
