For all paid campers kindly fill out the attached Registration Form and have it signed by your Parent or Authorized Guardian. Also, attach a photocopy of their valid id (reflecting their signature on the waiver part). And pass it to your Church Youth Ministry Head/Leader.
Strict compliance is a must.
