5. Certificate of Non-UNIFAST (1st Semester 2025–2026, from school)

6. School ID with Valid Sticker (1st Semester 2025–2026, from school)

7. Certificate of Indigency (Barangay)

8. Previous Grades (2nd Semester 2024-2025, from school)
