Fire Safety Maintenance Report (FSMR)

A FSMR is a written report prepared by the building owner, his/her fire safety practitioner or authorized representative. The FSMR is a compilation of the Fire and Life Safety System maintenance and testing records kept by the building or establishment, as a prerequisite for the issuance of a Fire Safety Inspection Certificate (FSIC) for Business or Mayor’s Permit renewal, Certificate of Annual Inspection , Permit to Operate, Philhealth Accreditation for Hospitals, DOH License to Operate and other permits or licenses being issued by other relevant government agencies.
