D. 1 photocopy of Transcript of Records and Diploma
E. 1 photocopy of Certificate of Eligibility, Board Rating, License and PRC card
F. 1 photocopy of Seminars/ trainings attended
G. 1 photocopy of Certificate of Experience/Employment (if any)
