Kami po ay nagsasagawa ng MASTER LISTING para po sa mga interesadong mapabakunahan ng HPV Vaccine ang inyong nga anak na babae na may edad na 9 hanggang 14 na taong gulang.

Ang HPV vaccine o Human Papillomavirus Vaccine ay mas kilala bilang "Anti-Cervical Cancer Vaccine.

Mangyaring paki-sagutan po ang google form na naka-post po dito. Ito po ay magsisilbing basehan ng mga mabibigyan ng libreng bakuna kapag dumating na po ang mga HPV vaccine.
