Kung ipapakuha sa authorized person,, narito ang mga kailangang dokumento:
- 2 xerox copies ng valid ID ng pensioner
- 2 xerox copies ng valid ID ng authorized person
- 2 xerox copies ng Authorization Letter mula sa Barangay na may pirma ng pensioner at may valid reason.

Sa mga kukuha po ng kopya ng authorization letter na fifill-upan ng pensioner ay mangyari lamang pong hanapin si Gng. MARGIE DELA CRUZ sa Barangay Hall ng ating barangay o kaya naman po ay maaaring itext o tawagan siya sa numerong 09362639241.
