Bawat komunidad na nabago, bawat mamamayan na nabigyan ng pag-asa—ito ang tunay na tatak ng KALAHI-CIDSS. Sa ilang dekada ng pagpapatupad ng programa, libu-libong Pilipino ang nakaranas ng konkretong pagbabago sa kanilang pamumuhay bunga ng sama-samang pagkilos at partisipasyon ng komunidad.

Ngayon, ating bigyang pugay ang mga Tatak KALAHI-CIDSS awardees—mga komunidad na sa tulong ng programa ay matagumpay na nakumpleto at napanatili ang mga proyekto na nagdala ng positibong epekto sa loob ng higit walong taon. Ito ay patunay ng mahusay, maayos at bukas na pamamahala ng mga community leaders na naglaan ng kanilang oras, puso at dedikasyon para sa kinabukasan ng kanilang nasasakupan.

Ang bawat tagumpay ng komunidad ay patunay na sa tulong ng KALAHI-CIDSS, nagkakaroon ng boses ang mga tao at naitataguyod ang pangmatagalang pag-unlad para sa lahat.

The Sangguniang Bayan is inviting all CSOs that are operating within the municipality to apply for accreditation. Attendance to the 𝗖𝗦𝗢 𝗖𝗼𝗻𝗳𝗲𝗿𝗲𝗻𝗰𝗲 𝗼𝗻 𝗝𝘂𝗹𝘆 29, 2025 (𝗧𝘂𝗲𝘀𝗱𝗮𝘆), (1𝗣𝗠) at the Women’s Development Center, Alegria, Surigao del Norte is required before applying for accreditation.

Please submit two (2) copies each of the following to apply for Accreditation of your Organization:
👉1. Letter of Application
👉2. Duly accomplished Application Form for Accreditation
👉3. Duly approved Board Resolution signifying intention for accreditation for the purpose of representation in a local special body
👉4. Certificate of Registration or existing valid Certificate of Accreditation from any NGA (or, in the case of IPOs, certification issued by NCIP)
👉5. List of current Officers
👉6. For CSOs that have been in operation for at least one (1) year, Minutes of the Annual Meetings of the immediately preceding year, as certified by the organization’s board secretary, or Certification from the board secretary certifying the annual meeting’s conduct, including the date, location, attendees, and agenda
👉7. For CSOs that have been in operation for at least one (1) year, the Annual Accomplishment Report for the immediately preceding year
👉8. For CSOs that have been in operation for at least one (1) year, a Financial Statement, at a minimum, signed by the executive officers of the organization, also of the immediately preceding year, and indicating therein other information such as revenue, expenses, and the source(s) of funds
👉9. Constitution & By-laws
👉10. Proof of registration or accreditation from the registering or accrediting government agencies.

2️⃣ Tibuok nga napunan nga Form sa Aplikasyon alang sa Akreditasyon
3️⃣ Board Resolution nga nagpakita sa tumong nga moaplikar alang sa akreditasyon (alang sa representasyon sa local special body)
4️⃣ Sertipiko sa Rehistro o valid nga Sertipiko sa Akreditasyon gikan sa bisan unsang NGA (o NCIP para sa IPOs)
5️⃣ Listahan sa kasamtangang mga Opisyal
6️⃣ Minutes sa Annual Meeting sa milabay nga tuig, o Sertipikasyon nga gipirmahan sa Board Secretary
7️⃣ Annual Accomplishment Report sa milabay nga tuig
