WHEN: March 30, 2026 at 8:00 AM - 10:00 AM (HALF DAY RA)

WHAT TO BRING!!!!:

A. Personal/Grantee :
1. Original & One (1) Photocopy of their OSCA IDs.

B. Grantee with Health and Medical Condition:
1. Original & Photocopy of any government issued ID of the authorized representative;
2. Original & photocopy of OSCA ID of grantee;
3. Original authorization letter; indicating the name of the grantee, name of the authorized representative and his/her relationship to the grantee, and reason of inability to personally claim the cash gift;
4. Medical Certificate for Grantee who are admitted on the day of payout or Barangay Certification for grantee who are bedridden and duly signed by the BHW and Barangay Captain attesting the grantee has an existing medical condition;
5. Proof of life: A full-body photo holding the latest 2026 calendar, showing the current month, printed on A4-sized bond paper.

C. DECEASED GRANTEE:
1. Original copy/certified true copy/authenticated copy of the death certificate as issued/certified/authenticated by the PSA or the Local Civil Registry;
2. Original and Photocopy of any valid government issued ID of the nearest surviving relative or claimant;
3. Original and Photocopy of proof of relationship such as birth certificate, Marriage Contract of any document that establishes the relationship of the nearest surviving relative with the deceased grantee;
4. In case there are numerous nearest surviving relatives (e.g. children), a Special Power of Attorney (PSA) (NCSC Form) shall be executed authorizing one of them to receive the benefits in behalf of all of them;

5. The claiming nearest surviving relative must also execute a warrant and release from liability form (NCSC form).

📢 ANNOUNCEMENT 📢

The Provincial Government of Surigao del Norte, through the Provincial Social Welfare and Development Office (PSWDO), will conduct the Medicine Assistance Program (MAP) Payout for all qualified Senior Citizens aged 80 years old and above in the Municipality of Socorro, Surigao del Norte.

🗓 Date: March 6, 2026
📍 Venue: Municipal Women’s Center, Socorro, Surigao del Norte

All qualified beneficiaries are advised to proceed to the venue on the scheduled date.

Note: Ang namatay na beneficiaries from July 2025 up to recent date pwede pa maka-claim paki-andam sa Death Certificate copy.
For those beneficiaries na nag-out of town pwede mag present nan medical certificate and authorization with ID ang mag claim
but if ang reason is not medical amo waya diha during payout ang beneficiary, Special Power of Attorney and i present.

Ang Opesina sa MSWDO, magpahibalo sa mga senior citizen na ng birthday sa Ika 80, 85, 90,95 ug 100 gikan March 17- December 2024 nga ang katapusan na pagdawat sa application para sa ECA cash gift is karong March 31, 2026. Palihog basin naay injong Nanay, tatay, lolo, ug lola na waya pa maka apply, pwede pa kaajo.
Mao kini ang mga requirements:
1. Filled up application form
2. Birth certificate (original/ authenticated)
3. Whole body picture
4. Photocopy sa OSCA ID with 3 signature/thumbmark
