Paalala mula sa Philippine Statistics Authority (PSA) na manatiling alerto at iwasan ang pakikipagtransaksyon sa mga manloloko. Tandaan, mahalaga ang iyong kaligtasan at seguridad!

Kung mayroon kang nakitang kahina-hinalang mga aktibidad o impormasyon tungkol sa mga insidente ng fixing, iulat agad ito sa PSA.

MADAYAW NA BALITA!

Sa tanan natao ng Aug. 3, 1988 to March 18, 2004 na Illegitimate child (dili kasal ang ginikanan) pwede na ninyo madala ang apelyedo sa inyong amahan thru the amendment sa R. A.9255.

Para dugang kasayuran, adtoa lang ang inyo City Civil Registrar's Office.

R.A. 9255 (Affidavit to Use the Surname of the Father)

This Act provides a blueprint for the assignment of last names for illegitimate children. Under RA 9255, all illegitimate children born on or after March 19, 2004 may use their father’s last name provided that an Authority to Use the Surname of the Father (AUSF) and the father acknowledges the child as his are executed. (PSA)

UNO ANG REQUIREMENTS?
(If registered na ang bata sa LCR)

1. PSA copy of birth certificate of the child

2. Affidavit of Acknowledgment/Admission of Paternity
●Ang amahan ang mag-execute

3. Affidavit to use the Surname of the Father
●Ang mama ang mag-execute
● If ang bata nag-edad 7-17 years old, ang bata ang mag-execute with sworn attestation of the mother

4. Isa ka dokumento nga nagpakita sa relasyon sa mag-ama. Example: Baptismal, SSS Forms, MDF sa Philhealth, etc.

Requirements on Delayed Registration
1. Negative Result from the PSA
2. Affidavit of 2 disinterested persons
3. Valid ID
4. Marriage Certificate of Parents (if may kasal ang ginikanan)
5. Duha (2) ka supporting docs nga nagpakita ug date of birth ug place of birth of the child.

Kung walay kasal ang ginikananan, the ff.are the addt'l Requirements
1. Affidavit to use the Surname of the Father
●kung ang bata 7 y.o., ang bata ang mag-execute sa AUSF with sworn attestation sa mother.
● kung ang bata 6 y.o. paubos, ang mother ang mag-execute sa affidavit.
●kung ang bata 18 y.o.and above, ang bata na ang mag-execute sa affidavit.

¤kini nga affidavit applicable lamang sa mga bata nga natao gikan March 19, 2004 until present.
