🔬 Clinical Case Presentation: Automated Analyzer Artifact vs. Manual Microscopy
📌 Case Title: Pseudothrombocytopenia and False Macrocytosis Secondary to Marked Rouleaux Formation
📅 Date of Analysis: 18-07-2026
1. Patient Laboratory Profile (Automated CBC)
The initial automated complete blood count (CBC) generated by the Erba Mannheim analyzer presented several significant flags and abnormal indices:ParameterPatient ResultReference Status / Flags
WBC9.42 \times 10^{9}\text{/L}Normal Range
RBC3.48 \times 10^{12}\text{/L}Decreased (Anemia)
HGB11.6\text{ g/dL}Mildly Decreased
MCV100.6 fLElevated (\uparrow Macrocytosis Flag)
PLT74 × 10⁹/LSevere Thrombocytopenia (\downarrow Flag)
MPV12.9 fLElevated (\uparrow Flag)
P-LCR52.6%Elevated (\uparrow Flag)
⚠️ Analyzer Interpretative Messages:
"Abn. PLT Distr" (Abnormal Platelet Distribution)
Platelet Histogram: Characterized by a flat, non-descending curve that fails to return to the baseline.
2. Peripheral Blood Smear (PBS) Findings
Upon identifying the distribution flags and conflicting indices, a manual peripheral smear review was performed. The microscopic examination revealed a distinct pathophysiological pattern across multiple fields:
Pronounced Rouleaux Formation: Red blood cells exhibited extensive, rhythmic stacking resembling alignment of coins throughout the fields.
True RBC Morphology: In thin areas, individual RBCs appeared predominantly normocytic, confirming that the macrocytosis was an artifact.
Platelet Masking/Distribution Alteration: Platelets were noted to be obscured or structurally clustered along the margins of the RBC stacks, directly affecting automated detection.
3. Pathophysiological Correlation (The Mechanism)
Why was the MCV falsely elevated?
Automated analyzers utilize electrical impedance or light-scattering technologies to measure cell volume. When severe Rouleaux formation is present, the tightly stacked RBC aggregates pass through the aperture simultaneously. The analyzer misinterprets these multi-cell clusters as individual, giant red blood cells, resulting in a falsely elevated MCV (100.6 fL) and a falsely decreased total RBC count (3.48 \times 10^{12}\text{/L}).
Why was the Platelet count falsely low?
The massive background crowding and stacking cause large platelets (indicated by the high MPV of 12.9 fL and P-LCR of 52.6%) to blend into or get trapped by the RBC matrices. This disrupts the lower discrimination thresholds of the platelet histogram, causing the analyzer to trigger the "Abn. PLT Distr" flag and underestimate the true platelet concentration.
4. Corrective Laboratory Actions & Recommendations
Saline Replacement Technique: To differentiate between true agglutination and protein-induced Rouleaux, a saline replacement test must be performed. If the stacking disperses after replacing the patient's plasma with normal saline, true Rouleaux is confirmed.
Clinical Chemistry Correlation: True Rouleaux formation is typically driven by high concentrations of plasma proteins. It is highly recommended to cross-reference this case with:
Total Protein & Albumin/Globulin (A/G) Ratio
Erythrocyte Sedimentation Rate (ESR) (expected to be significantly elevated)
Serum Protein Electrophoresis (SPEP) to investigate potential monoclonal gammopathies (e.g., Multiple Myeloma) or severe chronic inflammatory states.

Anemia affects millions of people, yet many don't realize they have it. It happens when your blood lacks enough healthy red blood cells to carry oxygen to your body's tissues, leaving you feeling "empty."
Watch out for these 4 Red Flags:
1️⃣ Constant Fatigue: Feeling exhausted even after a full night's sleep.
2️⃣ Pale Skin: A noticeable loss of color in your face, lips, or nail beds.
3️⃣ Shortness of Breath: Getting winded easily during simple tasks.
4️⃣ Cold Hands & Feet: Poor circulation caused by low oxygen levels. 🩸✨
