Orange County PACE by IIH Healthcare at a new PACE. We opened January 2022!

Orange County PACE (Program of All-Inclusive Care for the Elderly) helps frail, underserved seniors live happy, healthy lives independently in their own homes as an alternative to nursing care.

It's National PACE Month, and we've been celebrating all month long. 🎉 Today, we want to take a moment to thank the people who make it all possible: the team caring for our seniors day in and day out.

Thank you for your time, your care, and your heart. 💚

At PACE, your care is handled by a whole team, one that meets regularly to talk about you.

It's called the Interdisciplinary Team, or IDT, and it typically includes:
🩺 Primary care physician
👩‍⚕️ Nurse practitioner
💊 Pharmacist
🩹 Registered nurse
🧠 Behavioral health specialist
🦵 Physical, occupational, and speech therapists
🥗 Dietitian
🤝 Social worker
🎨 Activities coordinator
🏠 Home care coordinator
🚐 Transportation team
💚 Personal care aides

Each brings a different lens. Together, they build one coordinated care plan around the participant and adjust it as life changes.
You're not one person's patient. You're the whole team's priority.

Staff spotlight: Roehl Caragao 🚴
Join us in congratulating Roehl Caragao, Health Broker at Fresno PACE by IIH, who recently completed the Eastern Sierra Double Century, a 200-mile bicycle race through the Sierra Nevada with more than 10,000 feet of climbing, much of it at elevations above 7,000 feet.
The route includes the famous June Lake Loop, a stretch of mountain road that winds past four alpine lakes beneath towering Sierra peaks. Riders call it the highlight of the ride, but earning that view takes months of training and the determination to keep pedaling for 200 miles in a single day.
At IIH, that spirit feels familiar. Going the distance is what we do every day for our participants: showing up with commitment, endurance, and care for the long haul. Roehl, thank you for inspiring us on and off the bike. We're proud to have you on the team!

Gather 'round the campfire! 🔥 Our participants had a great time decorating these cozy campfire cookies. Sweet treats and even sweeter company.

Movement is one of the best things you can give your body at any age.

Walking around the block. A few minutes of stretching in the morning. Dancing in the kitchen. Gardening. Even standing up during commercials adds up.

Regular movement helps our participants sleep better, feel sharper, and hold on to the independence that matters most to them. Our activity and therapy teams build movement into daily routines because staying active is one of the surest ways to keep living fully.

What’s your favorite way to move?

Caring for an aging parent, spouse, or loved one is a full life of its own.

Behind so many of our participants is a family navigating appointments, coordinating care, and holding a lot of moving pieces together. It’s meaningful work, and it can also be a lot. We see you, and we’re here to help.

That’s where PACE comes in. Our care teams handle the coordination, scheduling, and transportation, along with many of the other pieces that keep care running smoothly. With that support in place, families and caregivers get to spend less time managing logistics and more time simply being together.

To the families and caregivers across Fresno, Bakersfield, and Orange County: we’re honored to care alongside you.

This Labor Day, we honor the workers whose dedication built this country. We recognize their effort, their perseverance, and the contributions that continue to strengthen our communities.
We’re especially grateful for our many team members whose work is care itself. They show up for our participants day in and day out.
And let us take a moment to recognize the family caregivers who carry their own labor, often quietly, to keep their loved ones safe and supported at home.
From all of us at PACE by IIH, we celebrate you with gratitude and appreciation.
