You can consent to have the form delivered to you electronically through the Georgia Medicaid website. If you choose to receive the form electronically, and you are already registered on the Georgia Medicaid website, please update your email address. You can provide your consent and review a list of Frequently Asked Questions by going to www.mmis.georgia.gov under the member information tab.
