Job Details

Scheduled visits are coordinated based on provider availability.

Exams are to be performed in OSHA and ADA compliant facilities.

NP’s will perform independent medical evaluations and medical record reviews. Evaluations only, not treatment.

Facility sites provided – Flexibility to work anytime between Sundays thru Saturdays– 7a.m. to 8p.m.

$65 per hour

Bonus incentives for total number of completed worksheets and/bilingual skills

Providers are advised to spend a minimum of 30 minutes with each Veteran

Job Requirements:

Nurse Practitioner: Masters’ Degree in Nursing or other education and experience allowing licensure by the State Board of Nursing as an Advanced Nurse Practitioner

Must have a license to practice in the state
Credentialing and degree obtainment will be verified upon hire

Back to School Picnic and Health Fair
The Garrett Morgan Community in conjunction with Professor Steve Sampson MSN, RN from Chamberlain School of Nursing and Larry Wilkes MSN, FNP-C from Wilkes Wellness Services, LLC are sponsoring a back to school picnic and health fair.
Join us September 6 at Mahalia Jackson Park 8407 S. Kerfoot Ave Chicago, Illinois 60620 from Noon until Dusk.
Services Provided:
Free book bags and school supplies
Food, talent show, music, games and for all.
Diabetes, Hypertension and Cholesterol screening.
Various health education opportunities

Wilkes Wellness is committed to participating/organizing health fairs monthly to help promote wellness and preventive care.

I need your help! This is me giving back. I am always in need of volunteers and donations for supplies. In the near future we will be holding fundraisers to help further this cause. I am counting on my fellow healthcare providers and my Lindblom alumni. To help save a life.

I want to thank Dr. Rajesh Iyengar from RITL Medical Services located 1701 W Monterey for donating supplies to Wilkes Wellness Services to participate in another health fair.

The health fair was a great success with the assistance of Brian Fondren, Bernice White and Steven Sampson we were able to educate and identify multiple people that are at risk for hypertension and heart disease. Also, We identified 4 people that had blood pressure that was greater than 200/110 at high risk for stroke.

I want to thank all the volunteers, nurses and nurse practitioners that made the Back to School Picnic and Health Fair held on August 31, 2014 a great success. Special thanks to the Lindblom Alumni Professor Steven Sampson Chamberlain College, Bernard Grimmage, and William Bryant FNP. Brian Fondren FNP promised to volunteer for an hour and stayed four hours.The staff of Wilkes Wellness Service, LLC and RITL Medical Services thank you for helping make this happen.

Through this hard work we were able to complete 100 Diabetes, Heart Disease, Hypertension and Stroke risk assessments. Also providing referrals and education in the African American community.
