🌿 WELLNESS WEDNESDAY: What Does a Case Manager Actually Do?

When life feels complicated, figuring out where to start can sometimes be one of the hardest parts.

That’s where case management can help.

At Rising Well, case managers work alongside individuals to understand their needs, identify challenges, connect them with resources, and coordinate services that can support their behavioral health, recovery, wellness and stability. Rising Well’s supportive services include case management and care coordination, community resource referrals, preventive wellness education and support for long-term stability.

So, what can case management look like in everyday life?

🧭 Figure out where to start — Talk through needs, challenges and goals and help identify appropriate next steps.

🤝 Connect with services & resources — Help identify behavioral health, healthcare and community resources that may meet an individual’s needs.

🔗 Coordinate care — Help individuals navigate different providers, services and supports so care feels more connected.

🏡 Address barriers to stability — Identify challenges affecting everyday life and connect individuals with available community resources and supportive services.

📋 Navigate complicated systems — Help make services, referrals and next steps easier to understand and manage.

💜 Work toward greater stability & wellness — Provide ongoing support and care coordination as an individual’s needs and goals evolve.

Think of a case manager as someone who helps connect the pieces of care—working alongside you to identify what you need, understand your options and navigate the resources and services that can help.

Case management isn’t about making decisions for someone. It’s about helping people feel supported, informed and connected as they work toward their goals.

💜 Sometimes the next step is simply having someone help you figure out what comes next.

💜 MENTAL HEALTH MONDAY: Behavioral Health Doesn't Fit Into One Box.

Mental health and substance use are often discussed separately.

For millions of people, they aren't separate experiences.

The newly released 2025 National Survey on Drug Use and Health examines mental illness, substance use disorders—and importantly, where the two overlap.

The same national data show the scale of both needs: 54.6 million adults experienced a mental illness in the past year, while 44.6 million Americans age 12 or older experienced a substance use disorder.

Behind those numbers are people whose needs may involve much more than a single diagnosis.

Mental health. Substance use. Housing. Physical health. Relationships. Transportation. Financial stability. Recovery.

They can all intersect.

💚 That's why Rising Well approaches behavioral health through a whole-person lens, bringing together mental health and substance use treatment, case management, recovery support, supportive housing and connections to resources.

People don't experience life's challenges in separate boxes.

Care shouldn't treat them that way either.
