Help shape the health of our community!

Northwestern Medicine is conducting a Community Health Survey to learn what health issues matter most to local residents. You do NOT need to be a Northwestern Medicine patient to take the survey.

Your answers can also help the DeKalb County Health Department (DCHD) learn more about our community’s health needs and help guide public health planning through our Community Health Needs Assessment and IPLAN process.

Your voice matters to public health! If you’re 18+ and live in a community served by Northwestern Medicine, we want to hear from you!

Scan the QR code or use the survey link to take the survey by October 23 and please share it with others in your community: https://survey.alchemer.com/s3/8883886/northwestern-2026-community-health-needs-assessment-survey
