The Rhode Island Public Health Association (RIPHA), in partnership with RIDOH's Diabetes and Cardiovascular Health Program, is offering a grant of up to $5,000 to support a Screen, Test, and Refer (STR) project focused on identifying adults with prediabetes or at high risk for Type 2 diabetes and connecting them to Ready for Health, Rhode Island's National Diabetes Prevention Program.

Eligible applicants include healthcare organizations, Federally Qualified Health Centers, and pharmacies serving Medicaid beneficiaries.
