How can we continue building a healthier, more connected Houston together? One conversation, one encounter, and one act of care at a time.
Healthcare doesn’t begin at the clinic door. Through our Street Medicine program, we bring care directly to individuals experiencing homelessness across Houston, meeting people where they are with compassion, consistency, and dignity.
From wound care and medication support to behavioral health connections and pathways into ongoing treatment, every encounter is an opportunity to build trust, improve health outcomes, and strengthen connection to care.

At HHH, we see how isolation can impact health and connection, and how care, consistency, and community can begin to open the door to something new.
