đź©ş Connecting uninsured residents of Duval County to no-cost and low-cost Primary Care.

JaxCareConnect is a joint collaboration created by the local nonprofit safety net of free and charitable clinics to assist uninsured adult Duval County residents in finding a primary care medical home. In addition to establishing primary care, we evaluate the whole person and explore other barriers to health care in order to support best possible health outcomes. Navigating health care can be an overwhelming process. The JaxCareConnect team is here to compassionately serve our neighbors and provide guidance about access to a health care provider. Our Patient Health Advocates work individually with each client to connect them to a primary care doctor, navigate community resources, and provide ongoing support along their health care journey. JaxCareConnect aims to better inform uninsured neighbors about their health care options so that all Duval County residents have access to comprehensive primary care—regardless of insurance status or ability to pay. JaxCareConnect was created in collaboration with Agape Community Health Center, Community Health Outreach, Muslim American Social Services, Mission House, Sulzbacher, Volunteers in Medicine and We Care Jacksonville.

We are a nonprofit that connects uninsured Duval County residents to a primary care provider through our network of free and charitable clinics. We also help patients with access to food, reliable transportation to appointments, and many other resources!

We are proud to have participated in the Volunteers in Medicine Open House event last week! VIM is one of our amazing partner clinics serving employed members of our community who don't have health insurance. We are lucky to have VIM as an option we can provide for our employed patients looking to get set up with a primary care doctor. This event offered a tour of the clinic and featured many of their partnership programs, from yoga to art therapy. We highly encourage anyone looking for a primary care clinic to connect with VIM and participate in their wonderful programs!

"Hi everyone! I’m Alice, and I’m so excited to be joining JaxCareConnect as a Patient Health Advocate! I’m originally from South Florida and have a background in healthcare and patient advocacy. I’ve always loved being able to help people, and I’m excited to bring that experience into the nonprofit world and have the opportunity to make a difference in our community.

After being diagnosed with kidney cancer, Ms. Smith suddenly lost her Medicaid coverage and went more than a year without seeing a doctor. After multiple unanswered attempts to reapply for insurance, she felt hopeless and gave up on getting care.

When Ms. Smith finally reached out to JaxCareConnect, everything changed. She was quickly connected to doctors and support programs and was able to schedule appointments right away.

What stood out most was the ongoing care. Her Patient Health Advocate checked in regularly, offered encouragement, and made sure her needs were met.

“JaxCareConnect helped me when I was at my lowest,” Ms. Smith shared. “They got my health back on track when I didn’t think it was possible.”
