Foundations Group is the parent company of Foundations Medical Adult Day Care and Psychiatric Rehabilitation Program. Foundations is designed to meet the needs of individuals (ages 18 and older) who have co-occurring health issues including chronic illness, mental illness and substance dependence. Attending Foundations allows individuals to remain at home with their loved ones and in their community. Foundations PRP facilitates an individual's recovery, including the individual's ability to make decisions while creating opportunities regarding home, school or work and community.

🎃🍕 Pumpkin Pizza Fun 🧡

Our participants had a wonderful time getting creative in the kitchen by making their very own pumpkin-shaped pizzas! 🍕🎃 Everyone enjoyed choosing their favorite toppings and adding their own personal touch to their delicious creations. From smiles to laughter, it was a fun-filled activity that brought everyone together for a little creativity, connection, and yummy fun! 😋❤️

Participants chose their favorite fresh fruits, mixed their own refreshing drinks, and enjoyed their creations. This hands-on Life Skills Group encouraged independence, following directions, making choices, creativity, and social interaction—all while having fun! 🥤💛
