I’ve heard the stories too many times! Patients and their families are often left in a state of despair! A few of the reasons for lack of coordination of care that I’ve seen: examples- - Someone at hospital forgot to fax a referral to home health -Homehealth states they did not receive the referral -Someone at hospital asked the patient only if they wanted home health, pt refused because they thought they had to pay for it -Someone in hospital assumed that insurance would not pay, so they never requested a service. -No one followed up, and family still waiting months for therapy that was not received. This doesn’t just pertain to homehealth and therapy, it’s across the board in health care! Notify your physician immediately if you have a need that was not addressed.
