Formed in 2014 by the merger of Foothills Care, The Solution Source, and LifeCare Family Services, our organization represents decades of combined experience and involvement in the community. We want to be your local partner in the treatment and care of those in need.

There are exciting changes happening at the Omni Family!

Over the course of the past year, we have been on a journey to redefine our brand with the end goal of clarifying the message about our organization and the work we do.
