St. Michael Medical Center (SMMC) is expanding its collaboration with Kitsap County fire agencies by investing in the Kitsap Fire CARES programs, enhancing their ability to support vulnerable community members across the region. As part of this collaboration, SMMC has hired an Advanced Registered Nurse Practitioner (ARNP) to work directly with the Kitsap Fire CARES teams, addressing clients’ unmet medical needs in their homes and throughout the community.

This investment significantly strengthens Kitsap Fire CARES’ ability to connect vulnerable community members with appropriate medical care before non emergent health concerns escalate to 911 calls, ambulance transport, or emergency department visits.

Heather Smith Merrithew, ARNP, will serve as the new advanced provider. In her role, she will collaborate closely with primary care providers, specialists, behavioral health professionals, and other community partners to ensure clients have access to the medical services they need. Heather will also deliver in-home and street based medical care for individuals facing significant barriers to traditional outpatient services—helping to improve access to care, reduce health disparities, and promote health literacy, healthier lifestyles, and overall well being.

“We are thrilled to have Heather join the team and fill this critical role in our community,” noted Alexandra McLaughlin, Interim Vice President of Operations at SMMC. “Our goal is to bring access to care closer to home for all area residents. In collaboration with Kitsap County Fire agencies and the CARES program, the new ARNP role will directly increase access to proactive and preventative care for all, especially some of our most vulnerable community members.”

The advanced provider position will be shared among the three Kitsap Fire CARES teams based out of Poulsbo Fire, Central Kitsap Fire & Rescue, and South Kitsap Fire & Rescue, and available through the Fire CARES referral process. This model mirrors an existing and successful collaboration between the Fire CARES teams and the Kitsap Recovery Center, which provides a Substance Use Disorder Professional to support the program. Together with dozens of other coordinated community partnerships, these relationships ensure seamless support while each organization maintains its dedicated scope of service.

These collaborations have been instrumental in the Fire CARES program’s success. In 2025, Fire CARES teams achieved reductions of 75%–86% in repeat 911 calls and 66%–83% in emergency department visits among the clients they served. This meaningful decrease not only lowers healthcare system strain and emergency medical costs but also contributes to improved outcomes and reduces the risk of life-threatening emergencies.

Working alongside the CARES teams, local healthcare providers, and community partners, Heather will help continue shaping and refining how mobile integrated healthcare services are delivered throughout Kitsap County. “We’re excited to welcome Heather to the team and look forward to the positive impact she will have on our community as we continue developing the CARES program and finding new ways to connect vulnerable community members with the care they need,” said Julie Rogers, Kitsap Fire CARES Program Administrator.

The Fire CARES teams and St. Michael Medical Center look forward to this next phase of collaboration—advancing a shared commitment to ensuring community members receive the right care, in the right setting, at the right time.
