HEC is proud to serve as the backbone convener of the West Sacramento Accountable Community for Health (ACH), bringing partners together to support community health.

In 2021, ACH partner Center for Land-Based Learning launched the Mobile Farmers Market through its Community Food Program, with collaboration from many ACH partners. Since then, the market has expanded to additional communities across Yolo County and Sacramento, bringing fresh food to more residents.

Community voice is shaping health priorities in Imperial County. Today, at the Community Health Assessment and Community Health Improvement Plan (CHA/CHIP) community priority areas workgroup and stakeholder meeting, the Imperial County ACH will share how it aligns and collaborates with the CHA/CHIP and other groups to support and grow the local healthcare workforce, and how its work fits within the broader ACH network. We’re proud to spotlight this work during California ACH Week.

Across California, Accountable Communities for Health are bringing health plans, local health departments, community-based organizations and residents together in shared health assessment and planning. Imperial County ACH’s work with the county’s CHA/CHIP is an important part of that statewide effort.

We're grateful to be part of another impactful Binational Health Week with our longtime partners at the Mexican Consulate!

Today, HEC joined the consulate and many other partners for the annual Binational Health Week Fair, helping connect community members with essential health and wellness resources, including financial education and other services that support their overall well-being. Here's to more partnerships that build healthier communities! 💚

Greeting our neighbors with smiles and fresh produce after a morning walk! 🥬

We appreciated seeing residents and families come together bright and early at Westfield Park in West Sacramento for Walk With Friends (WWF) early this week! From getting our steps in to sharing fresh produce and catching up with neighbors, it’s a fun way to start the day feeling good and staying connected.

El Driving Clean Assistance Program (DCAP) de California ayuda a los residentes elegibles del condado de Placer a dar el siguiente paso hacia la conducción de un vehículo eléctrico o híbrido enchufable.

A través del Driving Clean Assistance Program (DCAP), los solicitantes que cumplan con los requisitos pueden optar al programa Clean Cars 4 All, la opcíon de “desechar y reemplazar”, que permite deshacerse del vehículo actual y cambiarlo por uno híbrido o totalmente eléctrico.

Para calificar, los solicitantes deben:

- Tener 18 años o más (1 solicitante por hogar)
- Vivir en el condado de Sacramento
- Presentar la solicitud antes de comprar o arrendar un vehículo, y no puede haber participado en concursos de Vehículo eléctrico ligero associado de California Air Resource Board (CARB) como CVRP, CVAP, DCAP, o Clean Cars 4 All.

Si actualmente posee un vehículo y planea retirarlo a través del programa, debe ser del año 2010 o anterior.

Programas como DCAP facilitan la transición de las comunidades a un transporte más limpio y apoyan un futuro más saludable para todos. 🌱🔌

California's Driving Clean Assistance Program (DCAP) helps eligible Placer County residents take the next step toward driving an electric or plug-in hybrid vehicle.

Through the Driving Clean Assistance Program (DCAP), qualifying applicants can apply for Clean Cars 4 All, the Scrap & Replace program option which allows you to discard and upgrade your current vehicle for a hybrid or fully electric vehicle.

To be eligible, applicants must:
- Live in Sacramento County
- Be 18 or older (1 applicant per household)
- Meet income requirements

Programs like DCAP make it easier for communities to transition to cleaner transportation and support a healthier future for all.
