The conversation in Chicago was just the beginning.

NTSB safety recommendations turn lessons learned from accidents into actions that can prevent future tragedies. But those lessons can only save lives when they lead to action.

As Chairwoman Jennifer Homendy said, “The lessons we’ve learned from the past can save lives tomorrow only if our warnings are heeded.”

NTSB will continue working with rail workers, industry, regulators, and communities across the country and pressing for action on overdue recommendations to make rail safer for workers, passengers, communities, and road users.

Some opponents call impaired driving prevention technology a “kill switch.”

It’s not.

Impaired driving remains one of the leading causes of death on our nation’s roads. But technology can help prevent these crashes before they happen. NTSB has called for advanced impaired driving prevention technology in new vehicles; a safety measure Congress has already directed NHTSA to require.

Congress mandated action. The technology exists. Now it’s time to move forward.

As Chairwoman Jennifer Homendy said: “We can’t afford to wait any longer.”

The Senate’s passage of two pieces of bipartisan mental health legislation marks important progress for aviation safety.

NTSB has long championed reducing barriers that prevent aviation professionals from seeking mental health care. We thank the Senate for acting on this important issue.

During a shoving movement, rail equipment is pushed rather than pulled, which can limit the locomotive engineer’s view ahead.

Railroad workers may ride on the outside of moving equipment to provide “point protection,” exposing them to close clearances, falls, grade-crossing collisions, and derailments.

NTSB investigations show there are safer ways. We’re calling on the Federal Railroad Administration and the rail industry to prohibit this practice and eliminate unnecessary risks to rail workers.

Yesterday, NTSB welcomed members of the Department of War Joint Safety Council to headquarters to learn more about our approach to advancing transportation safety.

NTSB staff discussed how we manage our investigative workload and develop, issue, and track safety recommendations. DOW JSC visitors also toured NTSB laboratory facilities to learn more about the tools and expertise that support our investigations.

Chairwoman Jennifer Homendy joined NTSB staff in welcoming the council and emphasized our shared commitment to safety.

The visit provided an opportunity to share NTSB practices and exchange ideas on strengthening safety across organizations.

Ten years ago today, NJ Transit train 1614 crashed into Hoboken Terminal, killing one person and injuring 110.

Although the statutory deadline for full positive train control implementation was met in 2020, terminal exceptions left a critical safety gap. R-18-1 remains open with an unacceptable response, and NTSB continues to urge FRA to act on R-23-9 by working with railroads to remove terminal exceptions using improved PTC-related technologies.

NTSB investigations have identified critical safety issues across our rail system and led to recommendations aimed at preventing crashes, injuries, and deaths.

We’re highlighting the progress made, the risks that remain and the actions needed to strengthen rail safety for workers, passengers, road users and communities across the country, while keeping the people who work on America’s rails at the center of the conversation.

Because a safer rail system protects everyone, and every rail worker deserves to return home safely.

595 days since the DCA midair collision. Fifty safety recommendations. It’s time for action.

At the Air Line Pilots Association’s 70th Air Safety Forum, NTSB Chairwoman Jennifer Homendy called for urgent action on recommendations issued following our investigation of the Jan. 29, 2025, midair collision near DCA that claimed 67 lives.

During her keynote and a fireside chat with ALPA President Capt. Jason Ambrosi, the Chairwoman emphasized that every recommendation is aimed at preventing another tragedy, and every day without implementation represents an unacceptable risk to aviation safety.

NTSB Investigator-in-Charge Brice Banning also joined the forum’s “Honoring Legacy with Action” panel to discuss lessons learned from the investigation.

The families deserve more than promises. They deserve action to prevent future loss and heartbreak.
